The Episcopal Church requires the annual filing of a report by every Parish or other Congregation of this Church. This report is to be submitted to the Bishop of a Diocese not later than March 1, and to the national church not later than May 1. The report includes statistical and financial information from each Parish or Congregation, and is commonly known as the Parochial Report. It is the joint duty of the Rector, or other Member of the Clergy in charge, and the lay leadership of the congregation to prepare and file the Parochial Report. (Canon I.6.1)
The 2015 Parochial Report form, the General Instructions for completing it, and the Workbooks for Page 2 and Page 3 are available online (from the national church's website).
